Summary

We are seeking a Private Duty Nurse to join our compassionate team at Chosen Village. This role involves delivering personalized nursing care to patients in their homes, ensuring their comfort and well-being while supporting their medical needs.

Responsibilities

• Provide direct patient care, including monitoring vital signs and administering medications.
• Assist with IV infusions and other medical procedures as needed.
• Develop and implement individualized care plans based on patient assessments.
• Educate patients and families on health management and medication administration.
• Coordinate with healthcare professionals to ensure comprehensive patient care.
• Support patients with dementia through tailored care strategies.

Requirements

• Current nursing license in the state of Texas.
• Proven experience in primary care or home health nursing.
• Strong skills in patient assessment and medication administration.
• Experience with IV infusion and dementia care preferred.
• Excellent communication skills and ability to work independently.
